BODE Index (Palliative context)

BODE Index: Body-mass, Obstruction, Dyspnea, Exercise. Predicts 4-year mortality in COPD.

BMI ≥ 25 = 0 pts; BMI < 25 = 1 pt

≥50% = 0; 36-49% = 1; 20-35% = 2; <20% = 3 pts

≥251m = 0; 186-250m = 1; 101-185m = 2; ≤100m = 3 pts

Guidelines & Evidence

Clinical Details

Section 1

When to Use

When to Use

Prediction of mortality in end-stage COPD
Prognostic guidance for advance care planning in COPD populations
Identification of high-risk COPD patients
Guiding intensity of palliative interventions
Section 2

Formula & Logic

Components

B: Body-mass index
O: Airflow Obstruction (FEV1)
D: Dyspnea (mMRC scale)
E: Exercise capacity (6-minute walk distance)

Mortality Prediction

BODE 0–2< 5% mortality in 4 years
BODE 3–45–10% mortality in 4 years
BODE 5–610–20% mortality in 4 years
BODE 7–10> 20% mortality in 4 years

Pearls/Pitfalls

Key Advantages

Validated prognostic score in COPD populations
Integrates multiple domains (nutrition, physiology, symptom, function)
Useful for identifying end-stage COPD candidates for palliative focus
