Mechanism of Action

Acetic acid acts as a bactericidal agent by lowering pH, disrupting bacterial cell membranes, and inhibiting bacterial growth. It also has antifungal properties.

What the body does with it

MetabolismAcetic acid is metabolized via the tricarboxylic acid (TCA) cycle to carbon dioxide and water; minimal hepatic metabolism.
ExcretionAcetic acid 0.25% is a topical agent used for irrigation. Systemic absorption is negligible; any absorbed acetate is metabolized via the tricarboxylic acid cycle to CO2 and water. Less than 1% is excreted unchanged in urine. Fecal and biliary elimination are not relevant.
Half-lifeNot applicable for systemic half-life due to minimal absorption. If absorbed, acetate has a half-life of approximately 5-10 minutes due to rapid metabolism.
Protein bindingNegligible (<1%) due to rapid metabolism and small amount absorbed.
Volume of DistributionNot clinically relevant; with negligible systemic absorption, Vd is not defined for this formulation.
BioavailabilityTopical: not applicable (local effect). Oral/intravenous routes are not used; if ingested, acetate is rapidly metabolized.
Onset of ActionTopical irrigation: immediate upon contact with tissues (seconds to minutes).
Duration of ActionTopical irrigation: duration of action is limited to the period of contact; effects persist only while the solution is in contact with tissues. For antibacterial effect, activity is maintained during irrigation.

BreastfeedingAcetic acid 0.25% is a weak acid that is poorly absorbed systemically. Topical application is considered safe during breastfeeding. Avoid application to the breast if nipple fissures are present to prevent infant ingestion.

Clinical Tips & Counseling

Clinical PearlsAcetic acid 0.25% is used as a bladder irrigant to prevent and treat catheter-associated urinary tract infections (CAUTIs) by acidifying urine and inhibiting urease-producing bacteria. Use with caution in patients with mucosal irritation or known hypersensitivity. Monitor for hematuria, dysuria, or bladder spasms. Not for systemic use; discard unused portions due to lack of preservatives.
Patient AdviceThis solution is for bladder irrigation only and must not be injected or taken orally. · You may experience a mild burning sensation or bladder discomfort during irrigation. · Report any signs of allergic reaction (rash, itching, difficulty breathing) or severe pain immediately. · The solution is sterile; do not touch the container tip or reuse any leftover solution.
