ACETIC ACID 2% IN AQUEOUS ALUMINUM ACETATE
Clinical safety rating: caution
Comprehensive clinical and safety monograph for ACETIC ACID 2% IN AQUEOUS ALUMINUM ACETATE (ACETIC ACID 2% IN AQUEOUS ALUMINUM ACETATE).
Acetic acid exhibits antibacterial activity by lowering pH and disrupting bacterial cell membranes. Aluminum acetate acts as an astringent, precipitating proteins and reducing inflammation.
| Metabolism | Acetic acid is metabolized via the citric acid cycle; aluminum acetate is not significantly metabolized. |
| Excretion | Renal: >99% as acetate and aluminum; biliary/fecal: <1%. |
| Half-life | Not defined; systemic absorption minimal with topical/otic use; renal elimination of acetate rapid (minutes). |
| Protein binding | Acetate: negligible; aluminum: ~80% bound to transferrin and albumin. |
| Volume of Distribution | Not applicable for topical/otic; systemic absorption negligible. |
| Bioavailability | Topical/otic: minimal systemic absorption (<1%). |
| Onset of Action | Topical/otic: within minutes for antimicrobial effect. |
| Duration of Action | Short; requires repeated application every 4–6 hours for sustained effect. |
Soak a clean gauze or dressing in the solution and apply to the affected area for 15-30 minutes 3-4 times daily. For continuous wet dressing, apply and keep moist, changing every 2-4 hours.
| Dosage form | SOLUTION/DROPS |
| Renal impairment | No dosage adjustment required as systemic absorption is negligible. |
| Liver impairment | No dosage adjustment required as systemic absorption is negligible. |
| Pediatric use | Same as adult dosing: apply as a soak or wet dressing for 15-30 minutes 3-4 times daily. Use with caution in infants due to potential for increased absorption. |
| Geriatric use | No specific adjustment needed; use same dosing as adults. Monitor skin integrity due to increased fragility. |

Clinical note
Comprehensive clinical and safety monograph for ACETIC ACID 2% IN AQUEOUS ALUMINUM ACETATE (ACETIC ACID 2% IN AQUEOUS ALUMINUM ACETATE).
| Breastfeeding | Systemic absorption after topical application is negligible. The M/P ratio is unknown but expected to be low. Use with caution; avoid application to breast area to prevent infant ingestion. |
| Teratogenic Risk | Acetic acid 2% in aqueous aluminum acetate is a topical astringent solution. Systemic absorption is minimal; therefore, teratogenic risk is considered low. No well-controlled studies in pregnant women are available, but animal studies have not reported fetal harm. Use only if clearly needed, especially during the first trimester. |
■ FDA Black Box Warning
None.
| Serious Effects |
Hypersensitivity to components; perforated tympanic membrane (for otic preparations).
| Precautions | For external use only; avoid contact with eyes. Prolonged use may cause skin irritation or maceration. Not for use in patients with perforated tympanic membrane (otic use). |
| Food/Dietary | No significant food interactions reported. Avoid consuming alcohol in excess as it may impair immune function and healing. |
| Clinical Pearls | Acetic acid 2% in aqueous aluminum acetate (Burow's solution) is used as a topical astringent and anti-infective for acute otitis externa and weeping dermatoses. It restores the acidic pH of the ear canal (normal pH ~4-5) which inhibits Pseudomonas and fungal growth. Do not use in patients with perforated tympanic membrane due to risk of ototoxicity. For otitis externa, wick placement may improve delivery. Drainage from weeping lesions can be managed with wet-to-dry compresses. |

| Fetal Monitoring | No specific monitoring required due to negligible systemic absorption. Observe for local irritation or hypersensitivity reactions. |
| Fertility Effects | No known effects on fertility. Topical application unlikely to impact reproductive function. |
| Patient Advice | Use exactly as prescribed; do not apply to open wounds or broken skin unless directed. · Avoid contact with eyes; if contact occurs, rinse thoroughly with water. · For ear infections: lie on your side for 5 minutes after instilling drops to allow solution to penetrate. · Do not use if you have a perforated eardrum or ear tube (grommet) unless instructed by your doctor. · Discontinue use and inform your doctor if you experience severe burning, stinging, or worsening of condition. · Store at room temperature, away from heat and direct light, and discard any unused solution after course of therapy. |
