Mechanism of Action

Selective serotonin receptor agonist; interacts with 5-HT1B/1D receptors in cranial vessels to inhibit vasodilatation and neurogenic inflammation.

MetabolismPrimarily hepatic via monoamine oxidase (MAO-A); metabolites excreted renally.
ExcretionPrimarily hepatic metabolism with 80% renal excretion of inactive metabolites; 15% fecal elimination via bile; 5% unchanged drug in urine.
Half-lifeTerminal elimination half-life is 12-15 hours in healthy adults; prolonged to 20-30 hours in severe hepatic impairment and 15-20 hours in renal impairment (CrCl <30 mL/min).
Protein binding80-85% bound to albumin; minor binding to alpha-1-acid glycoprotein (5%).
Volume of Distribution0.8-1.2 L/kg, indicating extensive extravascular distribution, with highest concentrations in liver and kidneys.

Food/DietaryGrapefruit juice increases ALYACEN 777 plasma concentrations by inhibiting CYP3A4. Avoid grapefruit products. High-fat meals may delay absorption but do not reduce total exposure.
