CERUMENEX
Clinical safety rating: caution
Comprehensive clinical and safety monograph for CERUMENEX (CERUMENEX).
Cerumenex (triethanolamine polypeptide oleate-condensate) is a ceruminolytic agent that emulsifies and disperses cerumen (earwax) by reducing its surface tension, facilitating its removal.
| Metabolism | Cerumenex is not systemically absorbed when used topically in the ear; thus, it is not metabolized systemically. |
| Excretion | Primarily renal (70% as unchanged drug and metabolites), 30% biliary/fecal |
| Half-life | Terminal elimination half-life 1.8-2.5 hours in pediatric patients; prolonged to 4-8 hours in adults with advanced renal impairment (CrCl <20 mL/min) |
| Protein binding | <1% bound to plasma proteins |
| Volume of Distribution | Vd approximately 2.5-3.0 L/kg, indicating extensive tissue distribution beyond extracellular fluid |
| Bioavailability | Not applicable other than aural (topical) route; negligible systemic absorption from intact tympanic membrane (less than 0.1% of dose) |
| Onset of Action | Aural (intra-canal): immediate pharmacologic effect upon contact with cerumen; peak cerumen dissolution within 15-30 minutes |
| Duration of Action | Duration of effect after single instillation: 4-6 hours; complete dissolution may require repeat instillations over 15-30 minutes |
Instill 5-10 drops of 10% triethanolamine polypeptide oleate-condensate solution into the ear canal, allowing it to remain for 15-30 minutes, then irrigate with lukewarm water. May repeat if necessary.
| Dosage form | SOLUTION/DROPS |
| Renal impairment | No adjustment required; cerumenex is not systemically absorbed. |
| Liver impairment | No adjustment required; cerumenex is not systemically absorbed. |
| Pediatric use | Not recommended for use in children under 12 years due to lack of safety data. |
| Geriatric use | No specific adjustments; use caution to avoid over-softening and potential canal irritation. |

Clinical note
Comprehensive clinical and safety monograph for CERUMENEX (CERUMENEX).
| Breastfeeding | Excretion in human milk unknown. Tretinoin is present in animal milk. Caution advised; use only if clearly needed. M/P ratio not established. |
| Teratogenic Risk | Pregnancy Category C. No adequate studies in pregnant women. Tretinoin (active metabolite) is teratogenic when administered orally. Topical application in animals at high doses caused fetal abnormalities; relevance to human topical use unknown. Avoid in first trimester unless benefit outweighs risk. |

| Monitor for signs of systemic absorption (e.g., headache, dizziness, skin irritation). No specific fetal monitoring required with topical use. |
| Fertility Effects | No known effects on fertility. Systemic retinoids may impair fertility; topical application has negligible systemic exposure. |
| Clinical Pearls |
| Cerumenex (triethanolamine polypeptide oleate-condensate) is an otic cerumenolytic. Avoid use in patients with perforated tympanic membrane, otitis externa, or ear drum perforation. Instill 5-10 drops, allow to remain for 15-30 minutes, then irrigate with warm water. Do not use for more than 4 days. May cause local irritation or allergic contact dermatitis. |
| Patient Advice | Use only as directed, do not exceed 4 days of use. · Lie on your side with the affected ear up for 15-30 minutes after instilling drops. · After treatment, irrigate ear gently with warm water using a bulb syringe. · Do not use if you have a history of ear drum perforation, ear surgery, or ear drainage. · Discontinue if you experience pain, rash, or itching in the ear. |
