| Breastfeeding | Cetirizine is excreted into human breast milk. A study reported a milk-to-plasma ratio of 0.42 (range 0.25-0.63). The estimated daily infant dose via milk is 1.8-3.6% of the maternal weight-adjusted dose, which is considered low. The American Academy of Pediatrics considers cetirizine compatible with breastfeeding. Monitor infant for drowsiness or irritability. |
| Teratogenic Risk | Cetirizine is FDA Pregnancy Category B. Animal studies have not shown teratogenic effects. There are no adequate and well-controlled studies in pregnant women. First trimester data from the Swedish Medical Birth Registry (1995-2001) with 917 exposures showed no increased risk of major malformations. Second and third trimester use is not associated with fetal harm. However, due to the anticholinergic effects, use near term may cause neonatal effects such as irritability, tremors, and urinary retention. |

| Fertility Effects | Cetirizine has no known significant effects on human fertility. Animal studies at high doses showed no impairment of fertility. No clinical data suggest adverse effects on spermatogenesis or ovulatory function. |
| Children's Zyrtec Hives contains cetirizine, a second-generation antihistamine. Onset of action is within 1 hour; duration is 24 hours.
