CHLORAPREP ONE-STEP SEPP
Clinical safety rating: caution
Comprehensive clinical and safety monograph for CHLORAPREP ONE-STEP SEPP (CHLORAPREP ONE-STEP SEPP).
Chlorhexidine gluconate and isopropyl alcohol combination. Chlorhexidine disrupts bacterial cell membranes and precipitates cytoplasmic contents; isopropyl alcohol denatures proteins and dissolves lipids, providing rapid bactericidal activity.
| Metabolism | Chlorhexidine is poorly absorbed systemically; absorbed amounts are primarily excreted unchanged in urine. Isopropyl alcohol is metabolized by hepatic alcohol dehydrogenase to acetone and further to acetate and carbon dioxide. |
| Excretion | Chlorhexidine is primarily excreted via feces (>90%) as unchanged drug, with minimal renal excretion (<1%). A small amount is metabolized in the liver to inactive metabolites. |
| Half-life | The terminal elimination half-life in plasma is approximately 7-10 hours after topical application, but due to extensive tissue binding, sustained local concentrations persist for up to 48 hours. |
| Protein binding | Highly bound to plasma proteins (>90% bound to albumin and alpha-1-acid glycoprotein). |
| Volume of Distribution | Volume of distribution is 0.1-0.2 L/kg after topical application, indicating distribution primarily into interstitial fluid and tissues, with extensive local sequestration in skin and mucosa. |
| Bioavailability | The bioavailability of chlorhexidine is negligible (<0.1%) after topical application due to poor absorption across intact skin or mucous membranes; oral administration results in <1% systemic bioavailability. |
| Onset of Action | Within 30 seconds for bactericidal effect on skin; maximal antiseptic effect occurs within 2 minutes. |
| Duration of Action | Duration of action is 6-8 hours with a single application; cumulative effect with repeated use extends activity up to 24 hours due to persistent binding to stratum corneum. |
Apply 2% chlorhexidine gluconate and 70% isopropyl alcohol solution topically to the surgical site for 30 seconds using the applicator; allow to dry for 30 seconds to 2 minutes. Single-use only.
| Dosage form | SWAB |
| Renal impairment | No dose adjustment required as systemic absorption is negligible. |
| Liver impairment | No dose adjustment required as systemic absorption is negligible. |
| Pediatric use | For children <2 years: use with caution; apply minimal volume to avoid pooling. For children ≥2 years: same as adult dosing. |
| Geriatric use | Use with caution for thin or fragile skin; avoid excessive application. No dose adjustment required. |

Clinical note
Comprehensive clinical and safety monograph for CHLORAPREP ONE-STEP SEPP (CHLORAPREP ONE-STEP SEPP).
| Breastfeeding | Compatible with breastfeeding; negligible systemic absorption. M/P ratio not applicable due to minimal plasma levels. |
| Teratogenic Risk | No teratogenic risk expected; chlorhexidine and isopropyl alcohol are not absorbed systemically in significant amounts. No fetal risk in any trimester with topical use. |
| Fetal Monitoring | No specific monitoring required; standard skin antisepsis precautions apply. Avoid contact with mucous membranes. |

| Clinical Pearls | CHLORAPREP ONE-STEP SEPP contains 2% chlorhexidine gluconate and 70% isopropyl alcohol. It is a single-use antiseptic applicator for skin preparation prior to invasive procedures. Do not use on mucous membranes, open wounds, or near eyes. Allow to dry completely (typically 30 seconds to 2 minutes) before procedure to ensure antimicrobial efficacy and reduce fire risk. Avoid pooling of solution; use gentle pressure to apply in a back-and-forth motion for 30 seconds. Do not use in patients with known hypersensitivity to chlorhexidine or isopropyl alcohol. In neonates, avoid use due to risk of skin irritation and chemical burns. |
| Patient Advice | This solution is applied by a healthcare professional to your skin before a procedure to reduce germs. · Tell your healthcare provider if you have ever had an allergic reaction to chlorhexidine or alcohol. · Do not apply to open wounds, eyes, or inside your body. · You may feel a cool or stinging sensation during application; this is normal. · Keep the treated area dry and do not wash it off until after the procedure. · If you develop severe redness, itching, or blistering at the application site, contact your healthcare provider. |
