COLGATE TOTAL
Clinical safety rating: caution
Comprehensive clinical and safety monograph for COLGATE TOTAL (COLGATE TOTAL).
Triclosan inhibits bacterial enoyl-acyl carrier protein reductase (FabI), disrupting fatty acid synthesis and bacterial cell wall formation. Fluoride enhances remineralization and inhibits demineralization of tooth enamel.
| Metabolism | Triclosan is primarily metabolized via glucuronidation and sulfation in the liver. Fluoride is not metabolized but excreted renally. |
| Excretion | Primarily salivary clearance and swallowing; renal excretion of absorbed fluoride is approximately 50% of absorbed dose; unabsorbed fluoride excreted in feces; total systemic absorption is minimal (<1% of applied dose). |
| Half-life | Serum fluoride half-life is 2-4 hours after systemic absorption; clinically relevant only after ingestion of large amounts, as typical exposure from dentifrice is negligible. |
| Protein binding | Fluoride: minimal protein binding (<5%); triclosan: approximately 97% bound to plasma proteins. |
| Volume of Distribution | Fluoride: 0.5-0.7 L/kg; triclosan: approximately 2-3 L/kg, indicating extensive tissue distribution. |
| Bioavailability | Not applicable for topical toothpaste; systemic absorption from oral mucosa is negligible (<1%); oral ingestion yields bioavailability of approximately 50-60% for fluoride. |
| Onset of Action | Topical: immediate upon application for antiplaque effect; systemic effect on enamel remineralization requires minutes to hours of exposure. |
| Duration of Action | Topical: sustained release of triclosan (up to 12 hours) due to copolymer; fluoride retention in oral cavity for several hours; enamel remineralization persists after use. |
Apply a 1-inch strip of toothpaste to a soft-bristled toothbrush and brush teeth thoroughly for at least 2 minutes twice daily. Not for systemic use.
| Dosage form | PASTE |
| Renal impairment | No dose adjustment required as systemic absorption is negligible. |
| Liver impairment | No dose adjustment required as systemic absorption is negligible. |
| Pediatric use | Children under 2 years: Use only a smear of toothpaste (size of a grain of rice). Children 2-6 years: Use a pea-sized amount under adult supervision to minimize swallowing. Children over 6 years: Same as adult dose. |
| Geriatric use | No specific dose adjustment required; use same as adult dose. Ensure ability to perform oral hygiene and avoid swallowing. |

Clinical note
Comprehensive clinical and safety monograph for COLGATE TOTAL (COLGATE TOTAL).
| Breastfeeding | Triclosan and fluoride from toothpaste are not expected to be excreted in breast milk in significant amounts due to low systemic absorption. M/P ratio not available. Considered safe during breastfeeding. |
| Teratogenic Risk | Colgate Total contains triclosan (0.3%) and fluoride (0.24% as sodium fluoride). Topical use results in minimal systemic absorption. No teratogenic effects have been reported in animal studies or human data. Risk is negligible across all trimesters. |
