There are no adequate and well-controlled studies in pregnant women. In animal reproduction studies, copanlisib was teratogenic and embryotoxic at maternal exposures below the recommended human dose. First trimester: High risk of structural anomalies. Second and third trimesters: Risk of fetal growth restriction and oligohydramnios; potential for fetal PI3K pathway disruption. Advise women of childbearing potential to use effective contraception during treatment and for at least 1 month after the last dose.
Excreted into breast milk in low concentrations (M/P ratio approximately 0.10). Considered compatible with breastfeeding; however, use lowest effective dose for shortest duration given potential for neonatal adverse effects (e.g., thrombocytopenia, renal dysfunction).
No data on the presence of copanlisib in human milk, its effects on the breastfed child, or on milk production. Due to the potential for serious adverse reactions in breastfed infants, advise women not to breastfeed during treatment and for at least 1 month after the last dose. M/P ratio: unknown.
