Mechanism of Action
ACETAMINOPHEN AND IBUPROFEN

Acetaminophen is a centrally acting analgesic and antipyretic whose exact mechanism is not fully understood, but is thought to involve inhibition of cyclooxygenase (COX) in the brain and modulation of cannabinoid receptors. Ibuprofen is a nonsteroidal anti-inflammatory drug (NSAID) that non-selectively inhibits COX-1 and COX-2, reducing prostaglandin synthesis.

ACTIVELLA

Combination of estradiol, an estrogen, and norethindrone acetate, a progestin. Estrogens act by binding to nuclear estrogen receptors (ERα and ERβ), which then interact with estrogen response elements on DNA, leading to changes in gene expression that regulate growth, differentiation, and function of female reproductive tissues and other tissues. Norethindrone acetate is a progestin that induces secretory changes in the endometrium, reducing the risk of endometrial hyperplasia and carcinoma associated with unopposed estrogen therapy.

Acetaminophen: renal excretion of metabolites (glucuronide 55%, sulfate 30%, cysteine/mercapturate <10%); <5% unchanged. Ibuprofen: renal excretion of metabolites (conjugates) 90%; <10% unchanged; minor biliary/fecal.

Estradiol is primarily excreted in urine (∼50%) as glucuronide and sulfate conjugates, with ∼30% excreted in feces via biliary elimination. Norethindrone is excreted mainly in urine (∼60%) as metabolites, with ∼40% in feces.

FDA Black Box Warning

Acetaminophen may cause severe liver injury, including acute liver failure, at doses exceeding 4,000 mg/day. Ibuprofen: NSAIDs increase risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal. Risk increases with duration of use and in patients with cardiovascular risk factors. NSAIDs also increase risk of serious gastrointestinal adverse events including bleeding, ulceration, and perforation of stomach or intestines.

Estrogens increase the risk of endometrial cancer. There is an increased risk of cardiovascular events, breast cancer, and probable dementia with estrogen plus progestin therapy. Actively monitor for these events.

ACETAMINOPHEN AND IBUPROFEN

Combination product for acute pain; fixed-dose may exceed recommended daily acetaminophen limit if other acetaminophen-containing products are used. Onset of ibuprofen is 30-60 min, acetaminophen 15-30 min; duration 4-6 hours. Caution in renal impairment (ibuprofen) and hepatic impairment (acetaminophen). Avoid in third trimester of pregnancy.

ACTIVELLA

For patients with an intact uterus, estrogen must be combined with a progestogen (norethindrone acetate) to prevent endometrial hyperplasia. Initiate at the lowest effective dose for the shortest duration. Avoid in women with active thromboembolic disease, known or suspected breast cancer, or undiagnosed abnormal genital bleeding. Consider transdermal route if oral absorption is compromised or for migraine with aura.
