Mechanism of Action
ADDERALL 12.5

Adderall 12.5 is a combination of dextroamphetamine and amphetamine. It increases the levels of dopamine and norepinephrine in the central nervous system by inhibiting their reuptake and promoting their release from presynaptic neurons.

ATACAND HCT

ATACAND HCT is a combination of candesartan, an angiotensin II receptor blocker (ARB), and hydrochlorothiazide, a thiazide diuretic. Candesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively antagonizing the AT1 receptor, leading to vasodilation and reduced blood pressure. Hydrochlorothiazide inhibits the sodium-chloride symporter in the distal convoluted tubule of the nephron, increasing sodium, chloride, and water excretion, thereby reducing plasma volume and blood pressure.

Standard Dosing
ADDERALL 12.5

5-60 mg orally once or twice daily; immediate-release: initial 5 mg once or twice daily, increase by 5 mg weekly; extended-release: initial 20 mg once daily in the morning, increase by 10 mg weekly.

ATACAND HCT

One tablet orally once daily. Initial dose: 16 mg candesartan/12.5 mg hydrochlorothiazide. Titrate to maximum 32 mg candesartan/25 mg hydrochlorothiazide once daily.

Metabolism
ADDERALL 12.5

Amphetamine and dextroamphetamine are extensively metabolized in the liver via CYP2D6 and other pathways. The primary metabolites are 4-hydroxyamphetamine and 4-hydroxynorephedrine.

ATACAND HCT

Candesartan is primarily metabolized by hepatic O-deethylation via CYP2C9 to an inactive metabolite. Hydrochlorothiazide is not significantly metabolized and is excreted unchanged by the kidneys.

Approximately 30% of the dose is excreted unchanged in urine; the remainder is metabolized primarily via deamination and oxidation. Renal elimination of unchanged amphetamine is p H-dependent: acidic urine increases elimination, alkaline urine decreases it. Fecal excretion accounts for <5%.

Geriatric Dosing

Avoid acidic foods and beverages (e.g., citrus fruits, soda) within 1 hour of administration as they may decrease absorption. High-fat meals may delay absorption of extended-release formulations. Avoid caffeine and other stimulants. Grapefruit juice may increase amphetamine levels.

Avoid salt substitutes containing potassium chloride unless approved by your doctor. Limit high-potassium foods (e.g., bananas, oranges, tomatoes) if hyperkalemia risk is present.
