Clinical Essentials

ADDERALL 30
ALDORIL D30
Mechanism of Action
ADDERALL 30

Adderall contains mixed amphetamine salts that increase synaptic levels of dopamine and norepinephrine by inhibiting their reuptake and promoting release from presynaptic terminals.

ALDORIL D30

Aldoril D30 is a combination of methyldopa, a centrally acting alpha-2 adrenergic agonist that reduces sympathetic outflow, and hydrochlorothiazide, a thiazide diuretic that inhibits the sodium-chloride symporter in the distal convoluted tubule, decreasing plasma volume and peripheral resistance.

Indications
ADDERALL 30

Attention Deficit Hyperactivity Disorder (ADHD),Narcolepsy

ALDORIL D30

Hypertension

Pharmacokinetics

ADDERALL 30
ALDORIL D30
Half-Life
ADDERALL 30

Terminal elimination half-life: d-amphetamine 10-13 hours, l-amphetamine 13-15 hours; in adults (children: 6-8 hours). The longer half-life allows for once-daily dosing.

ALDORIL D30

Terminal elimination half-life of hydrochlorothiazide is 6-15 hours; methyldopa half-life is 1.8 hours (normal renal function). In renal impairment, half-life of both components is prolonged.

Metabolism
ADDERALL 30

Primarily hepatic via CYP2D6, with minor contributions from CYP1A2, CYP2B6, and CYP3A4.

ALDORIL D30

Methyldopa is metabolized by conjugation (catechol-O-methyltransferase) and hepatic sulfation; hydrochlorothiazide is not extensively metabolized and is excreted unchanged by the kidney.

Excretion
ADDERALL 30

Approximately 30-40% of a dose is excreted unchanged in urine; the remainder is metabolized primarily by oxidative deamination and aromatic hydroxylation. Biliary/fecal elimination accounts for less than 5%.

ALDORIL D30

Renal: approximately 50% as parent drug and metabolites; biliary/fecal: minimal, less than 5%.

Protein Binding
ADDERALL 30

Approximately 20-25% bound to plasma proteins, mainly albumin and alpha-1-acid glycoprotein.

ALDORIL D30

Methyldopa: <10% bound to plasma proteins; hydrochlorothiazide: 40-68% bound to albumin.

VD (L/kg)
ADDERALL 30

Vd: 3-4 L/kg (approximately 210-280 L for a 70 kg adult). This indicates extensive tissue distribution and penetration into the central nervous system.

ALDORIL D30

Methyldopa: Vd 0.2-0.3 L/kg (distributes into tissues, crosses placenta); hydrochlorothiazide: Vd 0.75-1.5 L/kg (extensively distributed, does not cross blood-brain barrier significantly).

Bioavailability
ADDERALL 30

Oral immediate-release: approximately 75-100%; oral extended-release: approximately 94% relative to immediate-release. Food does not significantly affect absorption but may delay peak concentration.

ALDORIL D30

Oral bioavailability of methyldopa is approximately 25% (variable, influenced by gut metabolism); hydrochlorothiazide bioavailability is 65-75%.

Food Interactions
ADDERALL 30

Avoid high-fat meals as they delay absorption; avoid acidic foods (e.g., citrus) and vitamin C supplements within 1 hour of dosing as they decrease absorption; limit caffeine and other stimulants to avoid additive cardiovascular effects.

ALDORIL D30

Food may decrease absorption of methyldopa. Avoid excessive intake of high-potassium foods (e.g., bananas, oranges) unless directed. Hydrochlorothiazide may cause potassium depletion; maintain adequate dietary potassium. Avoid natural licorice as it can worsen hypokalemia.

Pregnancy & Lactation

ADDERALL 30
ALDORIL D30
Teratogenic Risk
ADDERALL 30

Pregnancy category C. First trimester: No well-controlled studies, but potential for congenital malformations not definitively established. Second and third trimesters: Increased risk of premature delivery, low birth weight, and neonatal withdrawal symptoms (e.g., dysphoria, agitation, lassitude). Chronic use may lead to neonatal toxicity.

ALDORIL D30

First trimester: Limited data; no clear evidence of major malformations but methyldopa crosses placenta. Second and third trimesters: Associated with reduced placental perfusion; possible fetal bradycardia and neonatal hypotension. Hydrochlorothiazide may cause fetal/neonatal jaundice, thrombocytopenia, and electrolyte disturbances.

Lactation Summary
ADDERALL 30

Excreted in breast milk. M/P ratio unknown. Potential for stimulant effects in infant (e.g., irritability, poor feeding, insomnia). Caution advised; consider alternative feeding methods.

ALDORIL D30

Methyldopa is excreted in breast milk in low concentrations; M/P ratio approximately 0.2. Hydrochlorothiazide is excreted in minimal amounts; may suppress lactation. Consider risks versus benefits.

Pregnancy Dosing
ADDERALL 30

No established dosing guidelines. Due to increased plasma volume and clearance, dose may need titration to clinical effect, but avoid supratherapeutic doses. Use lowest effective dose.

ALDORIL D30

Methyldopa: Pregnancy-induced plasma volume expansion may require dose titration; monitor blood pressure and adjust accordingly. Hydrochlorothiazide: Often avoided in pregnancy due to volume depletion risks; if used, monitor electrolytes and renal function, no pharmacokinetic data necessitate routine dose adjustment.

Clinical Pearls
ADDERALL 30

For ADHD: start low, go slow; monitor weight and height in children; avoid late doses to prevent insomnia; check for abuse/diversion; screen for bipolar disorder and hypertension; consider urine drug screen before prescribing; avoid MAOIs within 14 days; use with caution in seizure disorders and glaucoma.

ALDORIL D30

ALDORIL D30 combines methyldopa (central alpha-2 agonist) and hydrochlorothiazide (thiazide diuretic). Monitor for orthostatic hypotension, especially at initiation. Taper not needed for methyldopa but discontinue if fever or liver dysfunction occurs. Interferes with urinary catecholamine measurements (false elevation). Hydrochlorothiazide may cause hyponatremia, hypokalemia, and hyperglycemia; check electrolytes and glucose periodically.
