AEROSEB-DEX vs CORTEF ACETATE
Comparing the clinical profiles, pharmacokinetic behaviors, and safety indices of these two therapeutic agents.
The combination product contains a corticosteroid (dexamethasone) which suppresses inflammation by inhibiting phospholipase A2, reducing prostaglandin and leukotriene synthesis, and a topical antibiotic (usually neomycin or polymyxin B) which inhibits bacterial protein synthesis or disrupts bacterial cell membranes.
Corticosteroid with anti-inflammatory and immunosuppressant activity; binds to glucocorticoid receptors, modulating gene expression and inhibiting phospholipase A2, thereby reducing prostaglandin and leukotriene synthesis.
2 puffs (100 mcg each) intranasally twice daily
Adult: 5-60 mg orally every 6-12 hours (hydrocortisone base equivalent), or 10-240 mg IV/IM every 12 hours (as hydrocortisone sodium succinate). Dose depends on severity and condition.

Terminal elimination half-life is 12-15 hours in adults with normal renal function; prolonged to 24-30 hours in severe renal impairment (CrCl <30 mL/min).
Plasma terminal half-life is approximately 1.5-2 hours. However, biologic half-life (duration of adrenal suppression) is 18-36 hours due to intracellular receptor binding.
Renal elimination of unchanged drug accounts for 30-40% of the dose; fecal/biliary elimination is 50-60% as metabolites. Less than 10% is excreted unchanged in feces.
Primarily renal as inactive metabolites; less than 5% unchanged. Biliary/fecal elimination is minimal (<2%).
