ALYFTREK vs SEGLENTIS

ALYFTREK (vutrisiran) is a transthyretin-directed small interfering RNA that binds to the 3' untranslated region of mutant and wild-type TTR mRNA, leading to its degradation via RNA interference, thereby reducing hepatic production of TTR protein and decreasing amyloid deposition.
SEGLENTIS is a fixed-dose combination of the opioid oxycodone and the opioid antagonist naltrexone. Oxycodone acts as a mu-opioid receptor agonist, providing analgesia. Naltrexone is intended to reduce the abuse potential of oxycodone by blocking opioid receptors when the drug is tampered with (e.g., crushed or chewed), but is sequestered in the core of the tablet and not released when taken orally as directed.
For patients with cystic fibrosis (CF) who are heterozygous for the F508del mutation in the CFTR gene and a minimal function mutation (F/MF genotypes): elexacaftor 200 mg/tezacaftor 100 mg/ivacaftor 125 mg orally, two tablets in the morning, and ivacaftor 150 mg orally, one tablet in the evening, approximately 12 hours apart. For patients homozygous for F508del (F/F genotypes): elexacaftor 200 mg/tezacaftor 100 mg/ivacaftor 125 mg orally, two tablets in the morning, and ivacaftor 150 mg orally, one tablet in the evening.
Subcutaneous injection: 300 mg (1.5 mL) once weekly. Administer in combination with oral capecitabine.

Terminal elimination half-life is approximately 72 hours after single dose and extends to ~120 hours at steady state due to dose-dependent elimination; allows once-weekly dosing.
The terminal elimination half-life of celecoxib is approximately 11 hours; for tramadol, it is about 6 hours, and for its active M1 metabolite, about 7 hours. Clinically, this supports twice-daily dosing for Seglentis (two tablets BID).
Primarily hepatic metabolism, with ~70% excreted in feces as metabolites and ~20% in urine (mostly as metabolites). <1% excreted unchanged in urine.
Seglentis (celecoxib and tramadol) is primarily excreted renally. Celecoxib is eliminated via hepatic metabolism (CYP2C9) with <3% excreted unchanged in urine; fecal excretion accounts for approximately 70% of an oral dose (as metabolites). Tramadol and its active metabolite (M1) are mainly excreted renally (about 90% of the dose, with 30% unchanged tramadol and 15% M1); the remainder is excreted fecally.
