Amikacin is an aminoglycoside antibiotic. There are no adequate and well-controlled studies in pregnant women. Aminoglycosides can cause fetal harm when administered to a pregnant woman. There is a potential for fetal ototoxicity and nephrotoxicity. First trimester: Risks unknown but avoid if possible. Second/Third trimester: Use only if clearly needed and if benefit outweighs risk; associated with irreversible bilateral congenital deafness when administered during pregnancy.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Aminoglycosides like amikacin cross the placenta. First trimester: No evidence of major malformations, but risk cannot be excluded. Second and third trimesters: Potential for fetal ototoxicity (eighth cranial nerve damage) and nephrotoxicity, especially with high doses or prolonged use. Avoid unless compelling indication.

Lactation Summary
AMIKACIN SULFATE IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Amikacin is excreted in human milk in low concentrations. The M/P ratio is approximately 0.15-0.5. Based on limited data, the dose to the infant is estimated to be <1% of maternal dose. Use with caution in nursing mothers; monitor infant for diarrhea, candidiasis, and potential allergic reactions. Consider the benefits of breast-feeding and the importance of amikacin to the mother.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Minimal excretion into breast milk (M/P ratio unknown but expected low). No reports of adverse effects in nursing infants from maternal amikacin use. Caution with infant renal impairment or premature infants due to potential accumulation. Use only if necessary.

Pregnancy Dosing
AMIKACIN SULFATE IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Pregnancy may alter pharmacokinetics due to increased volume of distribution and renal blood flow. However, specific dosing adjustments for amikacin in pregnancy are not well established. Monitor serum drug concentrations (peak and trough) to guide dosing, especially in patients with renal impairment or prolonged therapy. Use standard dosing with careful monitoring.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Increased renal clearance in pregnancy may lower serum levels; consider higher doses based on therapeutic drug monitoring. Adjust for renal impairment if present. Standard initial dosing: 15 mg/kg/day IV/IM divided q8-12h, with level-guided adjustments.

Avoid concomitant use with other nephrotoxic or ototoxic drugs (e.g., loop diuretics, vancomycin). Monitor peak (25-35 mcg/m L) and trough (<8 mcg/m L) serum levels to guide dosing and reduce toxicity risk. Extended-interval (once-daily) dosing is preferred in many patients; adjust for renal function using ideal body weight. In obese patients, dose based on adjusted body weight. Rapid infusion can cause neuromuscular blockade; use with caution in myasthenia gravis or concurrent neuromuscular blocking agents.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Amikacin is an aminoglycoside antibiotic with concentration-dependent bactericidal activity. Monitor peak (20-30 mcg/m L) and trough (<10 mcg/m L) serum levels to optimize efficacy and minimize toxicity. Adjust dose based on renal function (Cr Cl). Ototoxicity (vestibular and cochlear) and nephrotoxicity are dose-limiting; audiometry and renal function tests are mandatory. Extended-interval dosing (15-20 mg/kg once daily) is preferred for most indications. Avoid concurrent use with other nephrotoxic drugs (e.g., vancomycin, loop diuretics).

Safety Verification

Known Interactions

AMIKACIN SULFATE IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER Risks3
Amikacin + Masoprocol
moderate

"Amikacin, an aminoglycoside antibiotic, may competitively inhibit the renal tubular secretion and potentially reduce the clearance of masoprocol, a dicarboxylic acid derivative used as a chemotherapeutic agent. This interaction could lead to increased systemic exposure to masoprocol, elevating the risk of dose-dependent toxicities such as severe enteritis, myelosuppression, and hepatotoxicity. Given the narrow therapeutic index of masoprocol, even modest elevations in serum levels may result in clinically significant adverse outcomes."

Amikacin + Mycophenolic acid
moderate

"Amikacin, an aminoglycoside antibiotic, may competitively inhibit the tubular secretion of mycophenolic acid (MPA) in the renal proximal tubules, leading to reduced renal clearance of MPA. This interaction can result in elevated serum levels of MPA, increasing the risk of dose-related toxicities such as bone marrow suppression (leukopenia, thrombocytopenia), gastrointestinal disturbances, and increased susceptibility to infections. Patients receiving this combination should be closely monitored for signs of MPA toxicity, especially those with pre-existing renal impairment."

Metocurine + Amikacin
moderate

"Coadministration of Metocurine, a nondepolarizing neuromuscular blocking agent, with Amikacin, an aminoglycoside antibiotic, may result in enhanced and prolonged neuromuscular blockade. Aminoglycosides can impair acetylcholine release from presynaptic nerve terminals and reduce postsynaptic sensitivity, synergistically augmenting the effects of nondepolarizing agents. This interaction can lead to excessive muscle relaxation, including respiratory muscle paralysis, increasing the risk of apnea and postoperative respiratory depression."

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER Risks2
Lithium cation + Sodium chloride
moderate

"Lithium cation may increase the excretion rate of Sodium chloride which could result in a lower serum level and potentially a reduction in efficacy."

Sodium chloride + Tolvaptan
moderate

"The risk or severity of adverse effects can be increased when Sodium chloride is combined with Tolvaptan."
