The terminal elimination half-life is approximately 2-3 hours in adults with normal renal function. In neonates, it may be prolonged to 4-8 hours. In patients with impaired renal function, half-life can extend to 30-80 hours or more, necessitating dose adjustment based on creatinine clearance.

Amikacin is eliminated primarily by glomerular filtration. Approximately 94-98% of an administered dose is excreted unchanged in the urine within 24 hours in patients with normal renal function. Less than 1% is excreted in bile or feces.
Primarily renal (glomerular filtration); >90% excreted unchanged in urine. Biliary/fecal elimination is negligible (<1%).
