Aminosyn II 4.25% M in Dextrose 10% is a combination of amino acids and dextrose used for parenteral nutrition. Amino acids provide substrates for protein synthesis, while dextrose provides a source of calories. The amino acids undergo transamination, deamination, and incorporation into body proteins. Dextrose is metabolized via glycolysis, the Krebs cycle, and oxidative phosphorylation to produce ATP.
Intravenous infusion of 1.2 to 2.2 g amino acids per kg per day, adjusted to meet metabolic and nutritional requirements. Typical adult dose: initial infusion rate of 50 mL/hour, increasing to 125 mL/hour (4.4 g amino acids per hour) based on tolerance. Not for direct peripheral administration without supplementation of electrolytes and/or dextrose.
Intravenous infusion via central line. Adult dose: 500-2000 mL/day (equivalent to 21.25-85 g amino acids and 50-200 g dextrose) based on caloric and nitrogen requirements. Rate not to exceed 100 mL/hour initially, adjusted to maintain blood glucose <200 mg/dL.

The terminal elimination half-life of amino acids from the infusion mixture is not uniformly defined; however, individual amino acids have half-lives ranging from 10 to 30 minutes. For the mixture, the effective half-life is clinically considered to be approximately 1-2 hours, reflecting rapid metabolic clearance. Approximately 80-90% of infused nitrogen is recovered as urea in urine in patients with normal renal function.
