Direct thrombin inhibitor that reversibly binds to the active site of thrombin, blocking its interaction with substrates, thereby inhibiting fibrin formation and activation of coagulation factors V, VIII, and XIII.
Selective, direct, reversible inhibitor of factor Xa, blocking the conversion of prothrombin to thrombin, thereby reducing thrombin generation and thrombus formation.
1 mg/kg intravenous bolus followed by 0.1 mg/kg/hour continuous intravenous infusion for duration of procedure; alternatively, 0.75 mg/kg intravenous bolus followed by 1.75 mg/kg/hour continuous intravenous infusion for up to 4 hours during percutaneous coronary intervention.

Terminal elimination half-life is 10-14 hours. In patients with creatinine clearance 15-50 mL/min, half-life is prolonged to approximately 17-20 hours.
Renal: ~90% unchanged; biliary/fecal: negligible (<1%)
Renal excretion accounts for approximately 50% of the administered dose. Fecal excretion accounts for approximately 40%. Biliary excretion is minimal.
