Mechanism of Action
ANTHIM

Oblimersen is an antisense oligonucleotide that inhibits the production of Bcl-2 protein, promoting apoptosis in cancer cells.

ARZERRA

Ofatumumab is a fully human monoclonal antibody that binds specifically to the CD20 molecule on B lymphocytes, resulting in complement-dependent cytotoxicity (CDC) and antibody-dependent cell-mediated cytotoxicity (ADCC) of CD20+ cells.

Standard Dosing

800 mg IV over 90 minutes, then 400 mg IV over 90 minutes at 2 and 4 weeks post-first dose.

ARZERRA (ofatumumab) for chronic lymphocytic leukemia (CLL): Initial dose 300 mg IV, then 1 week later 2000 mg IV weekly for 6 doses, then 2000 mg IV every 4 weeks for up to 4 additional doses. For relapsed CLL: 300 mg IV followed by 1000 mg IV on day 8, then 1000 mg IV on day 15 and day 22 of cycle 1, then 1000 mg IV on day 1 of cycles 2-6 (28-day cycles). ANTHIM (obiltoxaximab) is a monoclonal antibody. Embryo-fetal developmental studies in monkeys showed no adverse effects at doses up to 17 times the human dose. However, human data is limited. As a Ig G1 monoclonal antibody, it is expected to cross the placenta increasingly after the first trimester. The risk is likely low but cannot be excluded. Use only if clearly needed.

Based on its mechanism of action (B-cell depletion), there is a potential risk of fetal B-cell lymphocytopenia and impaired immune response. Data from animal studies are insufficient. The drug should be avoided during pregnancy unless the benefit clearly outweighs the risk.

Lactation Summary

It is not known whether obiltoxaximab is excreted in human milk. Monoclonal antibodies are typically excreted in breast milk at low levels with limited oral bioavailability due to gastrointestinal degradation. The M/P ratio is unknown. Caution should be exercised, but benefits of breastfeeding and maternal therapy should be considered.
