Artemether-Lumefantrine

Artemether is rapidly converted to dihydroartemisinin, which produces free radicals that damage parasite proteins and membranes. Lumefantrine inhibits heme detoxification in the parasite food vacuole.

NEVANAC

Nepafenac is a nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX) enzymes, primarily COX-2, reducing prostaglandin synthesis and thereby suppressing ocular inflammation and pain.

Excretion
Artemether-Lumefantrine

Primarily fecal (biliary) elimination of unchanged drug and metabolites; renal excretion is negligible (<1% for artemether and <0.1% for lumefantrine). Artemether is extensively metabolized by CYP3A4/5 to dihydroartemisinin, which is further glucuronidated and excreted in bile. Lumefantrine is metabolized by CYP3A4 to desbutyl-lumefantrine; both parent and metabolite are eliminated via feces.

NEVANAC

Nepafenac is extensively metabolized, primarily via hydrolysis to amfenac. Renal excretion accounts for approximately 85% of the administered dose, with about 13% excreted as unchanged nepafenac and amfenac in urine. Fecal elimination is minimal.

Bioavailability

Administer one drop three times daily starting 1 day prior to surgery, continuing on day of surgery and for 2 weeks postoperatively.
