Mechanism of Action
AZOPT

Carbonic anhydrase inhibitor; inhibits carbonic anhydrase II (CA-II) in ciliary processes, reducing aqueous humor secretion and intraocular pressure.

ACEPHEN

ACEPHEN (acetaminophen) is a para-aminophenol derivative with analgesic and antipyretic activity. Its mechanism involves inhibition of cyclooxygenase (COX) enzymes in the central nervous system, particularly COX-2, reducing prostaglandin synthesis. It has weak peripheral COX inhibition and minimal anti-inflammatory effect.

Acetaminophen has been associated with cases of acute liver failure, at times resulting in liver transplant and death. Most of the cases of liver injury are associated with the use of acetaminophen at doses that exceed 4,000 milligrams per day, and often involve more than one acetaminophen-containing product.

First trimester: avoid if possible; second/third trimesters: monitor for maternal acidosis.

Pregnancy Category C. First trimester: potential risk of neural tube defects and orofacial clefts (limited human data, animal studies show embryotoxicity). AZOPT (brinzolamide ophthalmic suspension) is a carbonic anhydrase inhibitor used for lowering intraocular pressure in ocular hypertension or open-angle glaucoma. Shake well before use; may cause transient blurred vision. Use with caution in sulfonamide allergy patients. Monitor for corneal edema and electrolyte disturbances in prolonged use.
