Betamethasone acetate and betamethasone sodium phosphate are corticosteroids that bind to the glucocorticoid receptor, leading to modulation of gene expression and suppression of inflammatory mediators such as prostaglandins and leukotrienes. They inhibit phospholipase A2, reduce cytokine production, and decrease immune cell migration and activation.
Exservan (riluzole) is a benzothiazole derivative that modulates glutamatergic neurotransmission. Its mechanism of action involves inhibition of glutamate release, inactivation of voltage-dependent sodium channels, and interference with neurotransmitter binding to excitatory amino acid receptors.
1-4 mg (of betamethasone base) IM or IV every 12-24 hours, tapering as clinically indicated.
Adults: 15 mg orally once daily in the morning; increase to 30 mg after 2 weeks if needed. Maximum 30 mg/day.

The terminal elimination half-life of betamethasone is approximately 6.5 hours (range 4-8 hours) in plasma. This corresponds to a biological half-life of 36-54 hours for anti-inflammatory effects due to receptor occupancy and downstream effects.
