Biorphen (phenylephrine) is a selective alpha-1 adrenergic receptor agonist causing vasoconstriction and increased blood pressure.
Brivaracetam is a high-affinity synaptic vesicle glycoprotein 2A (SV2A) ligand, binding to SV2A with 15- to 30-fold higher affinity than levetiracetam. Brivaracetam is excreted into human breast milk with a milk-to-plasma (M/P) ratio of approximately 1.0. Infant exposure estimated at 0.5-1% of maternal weight-adjusted dose. Monitor infant for sedation, poor feeding, and weight gain. Benefit of breastfeeding may outweigh risks with caution.
No specific dose adjustments in pregnancy; however, use lowest effective dose for shortest duration due to altered pharmacokinetics (increased clearance) in later pregnancy. Taper dose gradually to avoid maternal withdrawal.
Pregnancy can decrease brivaracetam concentrations by 30-40% due to increased clearance and volume of distribution. Monitor clinical response and consider therapeutic drug monitoring to maintain trough levels within non-pregnant target range (0.5-10 mcg/m L).
