Selective beta-1 adrenergic receptor antagonist; reduces cardiac output, heart rate, and renin release from kidneys.

Adults: Initial dose 2.5-5 mg orally once daily, titrate to 10 mg once daily; maximum 20 mg once daily.
Intravenous: For stable patients, an initial loading dose of 500 mcg/kg/min over 1 minute followed by a maintenance infusion of 50 mcg/kg/min for 4 minutes; if response is inadequate, increase maintenance infusion to 100 mcg/kg/min and repeat loading dose after 10 minutes. Terminal elimination half-life is 9–12 hours (mean 11 hours), allowing once-daily dosing. Half-life may be prolonged in renal impairment (creatinine clearance <40 mL/min) and in elderly patients.
Terminal elimination half-life is approximately 9 minutes (range 8–10 minutes). Clinically, the half-life is consistent with rapid offset of effect upon discontinuation; steady state is achieved within 30 minutes of continuous infusion.
Approximately 50% excreted unchanged in urine; remainder metabolized in liver to inactive metabolites, then renally excreted. Fecal excretion is negligible (<2%). Total renal clearance accounts for ~60-70% of elimination.
Primarily metabolized by red blood cell esterases; <1% excreted unchanged in urine. Elimination is not dependent on renal or hepatic function.
