BRINSUPRI is a novel oral cyclin-dependent kinase (CDK) inhibitor that selectively inhibits CDK4 and CDK6, thereby blocking phosphorylation of the retinoblastoma (Rb) protein and preventing G1-to-S phase cell cycle progression. This induces cell cycle arrest in cancer cells with intact Rb function.
Fosinopril is an angiotensin-converting enzyme (ACE) inhibitor that inhibits the conversion of angiotensin I to angiotensin II, reducing vasoconstriction and aldosterone secretion; hydrochlorothiazide is a thiazide diuretic that inhibits sodium and chloride reabsorption in the distal convoluted tubule, increasing electrolyte and water excretion.
4 mg orally once daily, with or without food.
1 tablet (10-20 mg fosinopril / 12.5-25 mg hydrochlorothiazide) orally once daily; maximum dose 80 mg fosinopril / 50 mg hydrochlorothiazide per day.

Terminal elimination half-life is approximately 20-30 hours in healthy adults, allowing once-daily dosing. In renal impairment (CrCl <30 mL/min), half-life may extend to >50 hours, requiring dose adjustment.
Fosinoprilat: 11.5-12 h (terminal half-life extended in renal and hepatic impairment); hydrochlorothiazide: 5.6-14.8 h (varies with renal function).
Primarily renal excretion as unchanged drug (70-85%) and minor fecal elimination (10-15%). Biliary excretion accounts for <5%.
