BUSULFAN vs CARBOPLATIN

Busulfan is a bifunctional alkylating agent that crosslinks DNA, primarily at guanine N7 positions, leading to DNA strand breaks and inhibition of DNA replication and transcription. It is cell cycle phase-nonspecific.
Carboplatin forms platinum-DNA adducts, causing intrastrand crosslinks and G2/M cell cycle arrest, leading to apoptosis.
1-4 mg/day orally for remission induction in CML; 0.8-1 mg/kg every 6 hours orally for 4 days as part of myeloablative conditioning with cyclophosphamide.
IV infusion over 15-60 minutes, target AUC 4-6 using Calvert formula (dose = target AUC × (GFR + 25)). Adults: typical initial dose 400 mg/m² every 4 weeks or AUC 5-6 every 3-4 weeks.

Terminal elimination half-life is 2.5 to 4 hours (mean ~2.6 hours) after oral administration; prolonged to 3-5 hours with high-dose regimens. Half-life may increase with hepatic impairment.
Terminal elimination half-life: 2.6-5.9 hours in adults with normal renal function. In patients with creatinine clearance <60 mL/min, half-life is prolonged (up to 17-26 hours). Clinically, dosing adjustments are required based on Calvert formula using glomerular filtration rate.
Renal (10-50% unchanged), hepatic metabolism (primarily via glutathione S-transferases) with metabolites excreted in bile and urine. Fecal excretion minimal (<5%).
Renal: ~65% of platinum excreted in urine within 24 hours, primarily as unchanged carboplatin; ~32% as metabolites. Biliary/fecal excretion is minimal (<6%).
