Initial: 25 mg PO 2-3 times daily; target dose: 50 mg PO 2-3 times daily; maximum: 450 mg/day. For heart failure: start 6.25-12.5 mg PO 3 times daily, titrate to 25-50 mg PO 3 times daily.
