CELONTIN vs ZONISAMIDE

Increases levels of gamma-aminobutyric acid (GABA) in the central nervous system, possibly by inhibiting GABA transaminase or enhancing GABA release; also reduces calcium influx into neurons, stabilizing neuronal membranes.
Anticonvulsant; blocks voltage-gated sodium channels and T-type calcium channels, reducing neuronal excitability and seizure propagation. Also weakly inhibits carbonic anhydrase.
300 mg orally three times daily, increased by 300 mg every 3-4 days as tolerated; usual maintenance dose 900-2400 mg/day in divided doses.
Oral, initial 100 mg daily, may increase by 100 mg every 2 weeks; maintenance 200-400 mg daily in 1-2 divided doses; maximum 600 mg daily.

Terminal elimination half-life: 40-60 hours in adults, 30-45 hours in children; prolonged liver disease or renal impairment may increase half-life.
Terminal half-life approximately 60-70 hours (range 50-80 hours) in adults; at steady state, half-life may be slightly longer. Clinical context: requires 2-3 weeks to achieve steady state.
Renal: approximately 40-60% as unchanged drug; hepatic metabolism accounts for the remainder, with metabolites excreted renally.
Renal: approximately 30% unchanged; remainder as glucuronide conjugate and reduced metabolite. Biliary/fecal: minimal (<5%).
