Cetirizine hydrochloride is a second-generation histamine H1-receptor antagonist. It acts by selectively and reversibly blocking histamine H1 receptors on effector cells (e.g., smooth muscle, endothelial cells, mucous glands), thereby inhibiting histamine-mediated allergic responses such as vasodilation, increased vascular permeability, bronchoconstriction, and itching. It does not prevent histamine release but antagonizes its effects.
Promethazine is a phenothiazine derivative that acts as a competitive antagonist at histamine H1 receptors, exerting antihistaminic, sedative, antiemetic, anticholinergic, and local anesthetic effects. Its antiemetic effect is mediated via blockade of dopamine D2 receptors in the chemoreceptor trigger zone.
5-10 mg orally once daily; maximum 10 mg/day.
IV: 25-50 mg every 4-6 hours; IM: 25-50 mg every 4-6 hours; PO: 25-50 mg every 4-6 hours; PR: 25-50 mg every 4-6 hours; Maximum: 300 mg/day.
