Loratadine is a long-acting second-generation antihistamine that selectively antagonizes peripheral histamine H1 receptors, thereby inhibiting the effects of histamine released from mast cells and basophils, reducing allergic symptoms.
Promethazine is a phenothiazine derivative that acts as a histamine H1 receptor antagonist, antiemetic, and sedative. Dextromethorphan is a cough suppressant that acts as an NMDA receptor antagonist and sigma-1 receptor agonist.
10 mg orally once daily
For cough and upper respiratory symptoms: 5 mL (containing promethazine hydrochloride 6.25 mg and dextromethorphan hydrobromide 15 mg) orally every 4 to 6 hours, not to exceed 30 mL in 24 hours.

The terminal elimination half-life of loratadine is 8-14 hours (mean 11 hours) in healthy adults; for the active metabolite descarboethoxyloratadine, half-life is 17-24 hours (mean 20 hours). This supports once-daily dosing.
Promethazine: 10-19 hours (mean 12 hours). Dextromethorphan: extensive metabolizers (CYP2D6) 3-5 hours; poor metabolizers 20-30 hours. Clinical context: accumulation with repeated dosing, especially in poor metabolizers.
Loratadine is primarily eliminated via hepatic metabolism, with approximately 80% of the dose excreted as metabolites in urine (40%) and feces (40%). Less than 1% is excreted unchanged in urine.
