FERNISONE is a corticosteroid that binds to the glucocorticoid receptor, leading to inhibition of phospholipase A2, decreased prostaglandin and leukotriene synthesis, and suppression of inflammatory mediators.
One enema (100 mg hydrocortisone in 60 mL) administered rectally once daily, preferably at bedtime, for 21 days or until clinical response.

Terminal elimination half-life: 18-24 hours in healthy adults. In elderly (age >65), half-life increases to 30-36 hours due to reduced renal function.
