CORTENEMA vs OTOBIONE

Corticosteroid that binds to the glucocorticoid receptor, modulating gene expression to inhibit phospholipase A2, reduce prostaglandin and leukotriene synthesis, decrease cytokine production, and suppress inflammatory cell migration and activation in the colonic mucosa.
OTOBIONE is a combination product containing ciprofloxacin (a fluoroquinolone antibiotic) and fluocinolone acetonide (a corticosteroid). Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, leading to bacterial cell death. Fluocinolone acetonide suppresses inflammation by binding to glucocorticoid receptors, inhibiting phospholipase A2, and reducing prostaglandin and leukotriene synthesis.
One enema (100 mg hydrocortisone in 60 mL) administered rectally once daily, preferably at bedtime, for 21 days or until clinical response.
