CORTENEMA vs TRIATEX

Corticosteroid that binds to the glucocorticoid receptor, modulating gene expression to inhibit phospholipase A2, reduce prostaglandin and leukotriene synthesis, decrease cytokine production, and suppress inflammatory cell migration and activation in the colonic mucosa.
TRIATEX (methotrexate) inhibits dihydrofolate reductase, blocking tetrahydrofolate synthesis and thereby interfering with DNA synthesis, repair, and cellular replication. It also has immunomodulatory and anti-inflammatory effects through adenosine-mediated pathways.
One enema (100 mg hydrocortisone in 60 mL) administered rectally once daily, preferably at bedtime, for 21 days or until clinical response.
Triatex (trianterene/hydrochlorothiazide) 37.5 mg/25 mg or 75 mg/50 mg orally once daily; may increase to maximum of 2 capsules daily.
