Adalimumab is a recombinant human monoclonal antibody that binds to tumor necrosis factor-alpha (TNFα) and blocks its interaction with p55 and p75 cell surface TNF receptors. It also modulates biological responses induced or regulated by TNFα, including adhesion molecules, chemotaxis, and matrix metalloproteinases.
Micardis HCT is a combination of telmisartan, an angiotensin II receptor blocker (ARB), and hydrochlorothiazide, a thiazide diuretic. Telmisartan selectively blocks the binding of angiotensin II to AT1 receptors in vascular smooth muscle and adrenal gland, leading to vasodilation and reduced aldosterone secretion. Hydrochlorothiazide inhibits sodium reabsorption in the distal convoluted tubule, increasing excretion of sodium and water, thereby reducing plasma volume.

Adalimumab 40 mg subcutaneously every other week, with or without methotrexate, for rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, and plaque psoriasis. For ulcerative colitis and hidradenitis suppurativa, day 1: 160 mg (four 40 mg injections in one day or two 40 mg injections per day for two days), day 15: 80 mg, then 40 mg every other week starting day 29. For uveitis, 40 mg every other week.

Theoretical risk of harm to the fetus due to TNF inhibition; however, placental transfer is minimal during first trimester but increases in second and third trimester. There is evidence of increased risk of infections in neonates exposed in utero during later pregnancy. Therefore, use is not recommended in the third trimester unless clearly needed.
