Clinical Essentials

DAYPRO ALTA
ADDERALL 20
Mechanism of Action
DAYPRO ALTA

Oxaprozin is a nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX-1 and COX-2), thereby reducing prostaglandin synthesis, which mediates inflammation, pain, and fever.

ADDERALL 20

Adderall 20 is a combination of amphetamine and dextroamphetamine, which are central nervous system stimulants. They increase the levels of norepinephrine and dopamine in synaptic clefts by inhibiting their reuptake and promoting their release from presynaptic neurons.

Indications
DAYPRO ALTA

Rheumatoid arthritis,Osteoarthritis,Juvenile idiopathic arthritis,Ankylosing spondylitis (off-label),Acute gout (off-label)

ADDERALL 20

Attention Deficit Hyperactivity Disorder (ADHD),Narcolepsy,Off-label: Treatment-resistant depression, obesity, cognitive enhancement

Standard Dosing
DAYPRO ALTA

Oxaprozin is administered orally. The usual adult dose is 1200 mg once daily. For osteoarthritis and rheumatoid arthritis, dosing can range from 600 to 1200 mg once daily. A starting dose of 600 mg once daily may be considered for patients with low body weight or milder disease.

ADDERALL 20

Initial: 5 mg orally once or twice daily; may increase by 5 mg increments at weekly intervals. Usual effective dose: 20-40 mg/day divided into 1-2 doses. Maximum: 40 mg/day (immediate-release); 60 mg/day (extended-release).

Special Populations

DAYPRO ALTA
ADDERALL 20
Renal Adjustments
DAYPRO ALTA

For patients with creatinine clearance (Cr Cl) of 50-79 m L/min: no dose adjustment is generally required, but monitor for adverse effects. For Cr Cl 30-49 m L/min: reduce dose by 50% or use 600 mg once daily. For Cr Cl <30 m L/min: use is contraindicated. End-stage renal disease (ESRD): avoid use.

ADDERALL 20

e GFR 15-29 m L/min: 50% of usual dose. e GFR < 15 m L/min: avoid use due to accumulation risk. Hemodialysis: not recommended.

Hepatic Adjustments
DAYPRO ALTA

Child-Pugh Class A (mild impairment): no dose adjustment needed. Child-Pugh Class B (moderate impairment): reduce dose by 50% or use 600 mg once daily; monitor closely. Child-Pugh Class C (severe impairment): use is contraindicated. No specific studies; caution advised.

ADDERALL 20

Child-Pugh Class A: no adjustment. Child-Pugh Class B: reduce dose by 50%. Child-Pugh Class C: avoid use.

Pediatric Dosing
DAYPRO ALTA

Not approved for pediatric use. Safety and efficacy have not been established in patients under 18 years. Avoid use in children and adolescents unless under expert guidance and with caution.

ADDERALL 20

Children 3-5 years: 2.5 mg orally once daily; increase by 2.5 mg weekly. Children 6 years and older: 5 mg once or twice daily; increase by 5 mg weekly. Maximum dose: 40 mg/day (immediate-release). Weight-based: 0.3-1.5 mg/kg/day (immediate-release).

Geriatric Dosing
DAYPRO ALTA

Elderly patients (≥65 years) are at increased risk for NSAID-related adverse effects, including GI bleeding, renal impairment, and cardiovascular events. Initiate therapy at the lowest effective dose (e.g., 600 mg once daily) and monitor renal function, blood pressure, and for signs of GI toxicity. Avoid use if possible in patients with high cardiovascular risk or history of GI ulceration.

ADDERALL 20

Initial: 2.5 mg once or twice daily; increase slowly by 2.5 mg increments at weekly intervals. Use lowest effective dose due to increased sensitivity and risk of cardiovascular adverse effects.

Food Interactions
DAYPRO ALTA

May be taken with food or milk to minimize gastrointestinal irritation. Avoid alcohol due to increased risk of GI bleeding. No specific food restrictions otherwise.

ADDERALL 20

High-fat meals can delay absorption of Adderall. Acidic foods (e.g., citrus fruits, juices) and vitamin C may decrease absorption; avoid within 1 hour of dosing. Caffeine and other stimulants may increase side effects. Alcohol should be avoided. Grapefruit juice may increase amphetamine levels, so limit or avoid.

Pregnancy & Lactation

DAYPRO ALTA
ADDERALL 20
Teratogenic Risk
DAYPRO ALTA

First trimester: NSAIDs are not associated with a major teratogenic risk, but avoid due to potential risk of miscarriage. Second trimester: Use only if clearly needed. Third trimester: Avoid after 30 weeks due to premature closure of ductus arteriosus and oligohydramnios. DAYPRO ALTA (oxaprozin) is contraindicated in third trimester.

ADDERALL 20

First trimester: Increased risk of premature delivery and low birth weight; possible association with cardiovascular malformations (limited data). Second/third trimester: Risk of fetal growth restriction, preterm birth, neonatal withdrawal syndrome (irritability, poor feeding), and persistent pulmonary hypertension. Chronic use may impair fetal development.

Lactation Summary
DAYPRO ALTA

Oxaprozin is excreted in human milk; M/P ratio is approximately 0.5. Due to potential adverse effects on infant, caution is advised. Use only if benefit outweighs risk, consider alternative agents.

ADDERALL 20

Excreted into breast milk; M/P ratio approximately 2.5–7.5. Relative infant dose estimated at 5–14% of maternal weight-adjusted dose. Potential for decreased appetite, insomnia, and growth suppression in breastfed infants. American Academy of Pediatrics recommends use only if benefit outweighs risk, with close monitoring.

Pregnancy Dosing
DAYPRO ALTA

In pregnancy, oxaprozin clearance may increase; however, no specific dose adjustment is recommended. Use lowest effective dose for shortest duration during first and second trimesters. Avoid in third trimester.

ADDERALL 20

Due to increased renal clearance and expanded plasma volume, total amphetamine exposure may decrease, potentially requiring dose increase (monitor clinical response). However, insufficient data to recommend fixed adjustments; individualize based on symptom control and tolerability.

Clinical Pearls
DAYPRO ALTA

Daypro Alta (oxaprozin) is a nonsteroidal anti-inflammatory drug (NSAID) with a long half-life (~40-50 hours) allowing once-daily dosing. Monitor for GI bleeding, renal impairment, and cardiovascular events. Use with caution in elderly and those with renal insufficiency. Avoid in patients with aspirin-sensitive asthma or NSAID allergy.

ADDERALL 20

Adderall 20 mg is a mixed amphetamine salt formulation (75% dextroamphetamine, 25% levoamphetamine). Monitor for cardiovascular adverse effects; consider baseline ECG in patients with cardiac risk factors. Avoid in patients with structural cardiac abnormalities, cardiomyopathy, or arrhythmias.
