DESOGESTREL AND ETHINYL ESTRADIOL vs ESTRAGUARD

Desogestrel is a progestin that suppresses gonadotropin release, inhibiting ovulation. Ethinyl estradiol is an estrogen that provides negative feedback on the hypothalamic-pituitary axis, further suppressing ovulation and altering cervical mucus and endometrial lining to reduce sperm penetration and implantation.
Estradiol, the active ingredient, binds to estrogen receptors (ERα and ERβ) in target tissues, modulating gene transcription and exerting estrogenic effects including endometrial growth, vasodilation, and bone protection.
One tablet (0.15 mg desogestrel/0.03 mg ethinyl estradiol) orally once daily for 21 days, followed by 7 placebo tablets, then repeat cycle.
0.1% cream: 2-4 g intravaginally once daily for 2 weeks, then 1-2 g once daily 1-3 times per week for maintenance. Estradiol vaginal ring: 2 mg releasing 7.5 mcg/24h, inserted vaginally every 90 days.

Desogestrel: terminal half-life 23-27 hours (active metabolite etonogestrel); clinically allows once-daily dosing. Ethinyl estradiol: terminal half-life 12-15 hours (range 10-20 hours) with biphasic elimination; supports daily administration.
The terminal elimination half-life of estradiol is approximately 13-20 hours following transdermal administration, allowing for twice-weekly dosing. Oral estradiol has a shorter half-life of 2-4 hours due to first-pass metabolism.
Desogestrel: primarily renal (approximately 60% as metabolites), 30% fecal. Ethinyl estradiol: primarily renal (approximately 40% as glucuronide conjugates), 60% fecal.
Estradiol and its metabolites are primarily excreted in urine (approximately 90-95%), with about 5% excreted in feces via bile. Less than 10% is excreted unchanged.
