Mechanism of Action

Dextrose provides glucose for cellular energy metabolism; sodium chloride corrects electrolyte imbalances; potassium chloride maintains intracellular potassium levels and repolarizes cell membranes.

Aminophylline is a complex of theophylline and ethylenediamine. Theophylline acts as a non-selective phosphodiesterase inhibitor, increasing intracellular cyclic AMP levels, leading to bronchodilation. Narrow therapeutic index; serum theophylline levels must be monitored to avoid toxicity. Risk of seizures, cardiac arrhythmias, and death, especially at high serum concentrations. Caution in patients with hepatic impairment, congestive heart failure, cor pulmonale, fever, and in the elderly. Drug interactions with cimetidine, fluoroquinolones, macrolides, oral contraceptives, and other CYP1A2 inhibitors can increase toxicity.

Teratogenic Risk

Dextrose, sodium chloride, and potassium chloride are essential nutrients; at physiological doses, they are not associated with teratogenic effects. High-dose potassium chloride may cause maternal hyperkalemia, which can lead to fetal arrhythmia. Trimester-specific risks: 1st trimester: no known structural teratogenicity. 2nd/3rd trimester: electrolyte imbalances may affect fetal homeostasis; hyperkalemia may induce fetal bradycardia or arrhythmia.

Not for use as a maintenance solution in pediatric patients due to hypotonicity risk.

This prolonged exposure can heighten the risk of gastrointestinal erosion, bleeding, or perforation, particularly in patients with pre-existing lesions or receiving high-dose potassium supplementation. Clinically, this interaction necessitates close monitoring for signs of gastrointestinal injury when these agents are coadministered."
