DOXIL (LIPOSOMAL)

Doxorubicin intercalates between DNA base pairs, inhibits topoisomerase II, and generates free radicals, leading to DNA damage and cell death. Liposomal encapsulation prolongs circulation time and alters biodistribution.

BEYFORTUS (nirsevimab) is a recombinant human monoclonal antibody that binds to the prefusion conformation of the respiratory syncytial virus (RSV) F protein, inhibiting viral entry into host cells by blocking the fusion of the viral envelope with the host cell membrane.

Doxorubicin hydrochloride liposome injection (DOXIL) is classified as Pregnancy Category D. There is positive evidence of human fetal risk based on adverse reaction data from investigational or marketing experience or studies in humans. Potential benefits may warrant use of the drug in pregnant women despite potential risks. First trimester: High risk of teratogenicity including major malformations (e.g., cardiovascular, neural tube defects). Second and third trimesters: Risk of fetal growth restriction, oligohydramnios, and neonatal myelosuppression. Use only if clearly needed and no safer alternative.

BEYFORTUS

BEYFORTUS (nirsevimab) is a human monoclonal antibody against respiratory syncytial virus. There are no adequate and well-controlled studies in pregnant women. In animal reproduction studies, no adverse developmental effects were observed in pregnant rabbits or cynomolgus monkeys at doses up to 10 times the human clinical exposure. However, because monoclonal antibodies are transported across the placenta in increasing amounts as pregnancy progresses (especially in the third trimester), potential fetal exposure may occur. Based on limited data, the risk of major birth defects and miscarriage is unknown but expected to be low due to the Ig G1 nature and lack of known teratogenic signal.

Lactation Summary
DOXIL (LIPOSOMAL)

Doxorubicin is excreted in human milk. The milk-to-plasma (M/P) ratio for doxorubicin is approximately 0.5 to 2.0 based on limited data. Because of the potential for serious adverse reactions in nursing infants from doxorubicin (e.g., myelosuppression, cardiotoxicity), discontinue breastfeeding during and for at least 3 months after the last dose of DOXIL.

BEYFORTUS

There are no data on the presence of nirsevimab in human milk, effects on the breastfed infant, or effects on milk production. Nirsevimab is a human monoclonal antibody (Ig G1) and is expected to be excreted into human milk in small amounts due to the high molecular weight and limited transfer via the neonatal Fc receptor. The M/P ratio has not been determined. The developmental and health benefits of breastfeeding should be considered along with the mother's clinical need for BEYFORTUS and any potential adverse effects on the breastfed infant from the drug or underlying condition.

Pregnancy Dosing

Pharmacokinetic data in pregnancy are limited; however, physiological changes (e.g., increased plasma volume, hepatic clearance) may alter doxorubicin exposure. No specific dose adjustment guidelines exist. Use the standard dose based on body surface area (BSA) while closely monitoring for toxicity. BEYFORTUS
