Mechanism of Action
DUZALLO

DUZALLO (allopurinol) is a xanthine oxidase inhibitor that reduces uric acid production by inhibiting the conversion of hypoxanthine to xanthine and xanthine to uric acid.

ALLOPURINOL

Allopurinol inhibits xanthine oxidase, the enzyme responsible for the conversion of hypoxanthine to xanthine and xanthine to uric acid, thereby reducing serum and urinary uric acid concentrations. It also inhibits de novo purine synthesis through feedback inhibition.

Management of signs and symptoms of primary or secondary gout (acute attacks, tophi, joint destruction, uric acid lithiasis, and/or nephropathy),Prevention of tumor lysis syndrome in patients receiving chemotherapy for leukemia, lymphoma, or solid tumor malignancies

Allopurinol is metabolized primarily by aldehyde oxidase to its active metabolite oxypurinol (alloxanthine), which also inhibits xanthine oxidase. Oxypurinol is further metabolized and eliminated renally.

DUZALLO

DUZALLO (elagolix/estradiol/norethindrone acetate) is a Gn RH antagonist combination product for management of heavy menstrual bleeding in premenopausal women with uterine leiomyomas. Monitor bone mineral density with prolonged use beyond 6 months; avoid in patients with osteoporosis risk factors. Contraindicated with strong CYP3A4 inhibitors and in pregnancy. Assess for mood changes and depression. Use effective non-hormonal contraception during treatment.

ALLOPURINOL

Start at low dose (100 mg/day) and titrate every 2-4 weeks to reduce risk of gout flare; check renal function before dosing and adjust accordingly; allopurinol hypersensitivity syndrome (AHS) is rare but life-threatening, discontinue immediately if rash or signs of hypersensitivity occur; avoid use with azathioprine or 6-mercaptopurine unless dose of these agents is reduced by 60-80%; monitor liver function tests periodically.
