Mechanism of Action
ELLENCE

ELLENCE (epirubicin) is an anthracycline cytotoxic antibiotic. It intercalates between DNA base pairs, inhibits topoisomerase II activity, and generates free radicals, leading to DNA damage and cell death.

PORTRAZZA

PORTRAZZA (necitumumab) is a recombinant human Ig G1 monoclonal antibody that binds to the epidermal growth factor receptor (EGFR), thereby inhibiting ligand binding and subsequent activation of EGFR, leading to inhibition of downstream signaling pathways involved in cell proliferation and survival.

Adjuvant therapy in patients with axillary node-positive breast cancer,Treatment of metastatic breast cancer,Off-label: treatment of ovarian cancer, gastric cancer, small cell lung cancer, and soft tissue sarcoma

PORTRAZZA

First-line treatment of metastatic squamous non-small cell lung cancer (NSCLC) in combination with gemcitabine and cisplatin.

Standard Dosing
ELLENCE

60-120 mg/m2 IV bolus or slow infusion on Day 1 every 21-28 days; or 20-30 mg/m2 IV daily for 3 days repeated every 28 days.

PORTRAZZA

PORTRAZZA (necitumumab) is administered intravenously at a dose of 800 mg over 60 minutes on days 1 and 8 of each 21-day cycle.

Based on its mechanism of action (EGFR inhibition), there is a risk of fetal harm, including developmental abnormalities and fetal loss. Women of reproductive potential should use effective contraception during treatment and for at least 3 months after the last dose.

Lactation Summary

Ellence (epirubicin) is an anthracycline chemotherapeutic agent. It is a vesicant; extravasation can cause severe tissue necrosis. Administer via a freely flowing IV line. Premedicate with antiemetics. Monitor for cardiotoxicity, which is dose-dependent and may be cumulative. Total lifetime dose should not exceed 900-1000 mg/m². Assess cardiac function (LVEF) before and during treatment. Urine may turn red for 1-2 days after administration. Avoid live vaccines.

PORTRAZZA

PORTRAZZA (necitumumab) is a human Ig G1 monoclonal antibody targeting EGFR. Prior to initiation, confirm EGFR expression in squamous non-small cell lung cancer. Premedicate with H1 antagonists to reduce infusion-related reactions. Monitor for hypomagnesemia, which can occur weeks after treatment; replete as needed.
