ETHINYL ESTRADIOL AND NORELGESTROMIN vs PREMPHASE (PREMARIN;CYCRIN 14/14)

Combination contraceptive: estrogen (ethinyl estradiol) suppresses gonadotropin release via negative feedback on pituitary; progestin (norelgestromin) thickens cervical mucus and inhibits ovulation.
PREMPHASE combines conjugated estrogens (PREMARIN) and medroxyprogesterone acetate (CYCRIN). Estrogens act by binding to nuclear estrogen receptors (ERα and ERβ), which regulate gene transcription and produce effects in tissues such as the endometrium, breast, and bone. Medroxyprogesterone acetate is a progestin that induces secretory changes in the endometrium and reduces the risk of endometrial hyperplasia associated with estrogen therapy.
One transdermal patch (releasing 0.035 mg ethinyl estradiol and 0.150 mg norelgestromin per 24 hours) applied once weekly for 3 weeks, followed by 1 week patch-free.
One tablet daily (conjugated estrogens 0.625 mg/medroxyprogesterone acetate 5 mg) for 14 days, followed by one tablet daily (conjugated estrogens 0.625 mg) for 14 days; continuous cycling. Oral administration.

Ethinyl estradiol has a terminal elimination half-life of approximately 13-27 hours (mean ~17 hours). Norelgestromin has a terminal half-life of about 28 hours. These half-lives support once-weekly dosing of the transdermal system, achieving steady-state by the second application.
Conjugated estrogens: terminal half-life 10–24 h (accumulation with daily dosing). MPA: terminal half-life 12–33 h (mean ∼17 h).
Ethinyl estradiol and norelgestromin are excreted primarily via urine and feces. Ethinyl estradiol undergoes extensive metabolism; about 40% is excreted in urine and 60% in feces as glucuronide and sulfate conjugates. Norelgestromin is metabolized to norgestrel and other metabolites; approximately 45% is excreted in urine and 35% in feces.
Conjugated estrogens and MPA are primarily excreted in urine (∼90% as glucuronide and sulfate conjugates) and feces (∼10% as unabsorbed drug and biliary metabolites).
