FAYOSIM vs ZYRTEC HIVES

FAYOSIM (plecanatide) is a guanylate cyclase-C (GC-C) agonist. It binds to GC-C receptors on the luminal surface of intestinal epithelial cells, activating the receptor and increasing intracellular cyclic guanosine monophosphate (cGMP) levels. Elevated cGMP stimulates chloride and bicarbonate secretion into the intestinal lumen, enhancing fluid secretion and accelerating gastrointestinal transit, thereby promoting bowel movements.
Selective histamine H1-receptor antagonist. Inhibits histamine-mediated vasodilation, capillary permeability, and smooth muscle contraction.
10 mg orally once daily
For chronic idiopathic urticaria, adults: 10 mg orally once daily. For intermittent symptoms, up to 10 mg once daily as needed.

12-16 hours in healthy adults; prolonged to 20-30 hours in moderate renal impairment (CrCl <50 mL/min) requiring dose adjustment.
The terminal elimination half-life is approximately 8.3 hours in healthy adults. In patients with renal impairment (CrCl < 40 mL/min), half-life can extend to 18–21 hours, necessitating dose adjustment.
Primarily renal elimination, 80% unchanged drug in urine; 15% biliary/fecal; 5% metabolized.
Cetirizine is primarily excreted renally as unchanged drug (approximately 70%). Fecal excretion accounts for about 10%. The remainder undergoes hepatic metabolism to inactive metabolites, which are also renally eliminated.
