Sulfamethoxazole is a sulfonamide that inhibits bacterial dihydropteroate synthase, preventing folate synthesis. Trimethoprim inhibits bacterial dihydrofolate reductase, blocking tetrahydrofolate production. The combination produces sequential blockade of folate metabolism, leading to bactericidal activity.
Competitive inhibitor of dihydropteroate synthase, blocking para-aminobenzoic acid (PABA) incorporation into dihydropteroic acid, thereby inhibiting bacterial folic acid synthesis.
800 mg orally every 12 hours for 5-7 days.
2-4 g orally initially, then 2-4 g every 4-6 hours, not to exceed 12 g/day; intravenous: 4-8 g/day in divided doses every 6-8 hours.
