Intravenous: 2 mg/kg every 12 hours; Oral: 20 mg twice daily.
Initial: 1 mg orally once daily; titrate by 1 mg every 3-5 days based on response and tolerability. Maximum: 8 mg once daily. Administer at bedtime.
