Hydrochlorothiazide inhibits the Na+-Cl- symporter in the distal convoluted tubule, increasing excretion of sodium, chloride, and water. Reserpine depletes catecholamines (norepinephrine, dopamine) from peripheral sympathetic nerve endings, reducing vascular tone and heart rate.
Lopressidone is an atypical antipsychotic that antagonizes dopamine D2 and serotonin 5-HT2A receptors, with higher affinity for 5-HT2A than D2, and also blocks alpha1-adrenergic and H1 histamine receptors.
Hydrochlorothiazide 50 mg orally once daily.
Oral: 5 mg twice daily, titrate as tolerated up to 20 mg twice daily. Maximum 40 mg per day.
