Pregnancy & Lactation

INJECTAPAP
AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER
Teratogenic Risk
INJECTAPAP

FDA Category C. Acetaminophen crosses the placenta. No evidence of teratogenicity in humans with standard doses. First trimester: limited data suggest no increased risk of major malformations. Second and third trimesters: chronic high-dose use may be associated with increased risk of childhood asthma and attention-deficit/hyperactivity disorder (ADHD). Overdose poses risk of maternal and fetal hepatotoxicity.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Aminoglycosides like amikacin cross the placenta. First trimester: No evidence of major malformations, but risk cannot be excluded. Second and third trimesters: Potential for fetal ototoxicity (eighth cranial nerve damage) and nephrotoxicity, especially with high doses or prolonged use. Avoid unless compelling indication.

Lactation Summary
INJECTAPAP

Acetaminophen is excreted into breast milk in low concentrations (M/P ratio approximately 0.91-1.42). Reported infant dose is less than 2% of maternal weight-adjusted dose. Considered compatible with breastfeeding. Use lowest effective dose for shortest duration.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Minimal excretion into breast milk (M/P ratio unknown but expected low). No reports of adverse effects in nursing infants from maternal amikacin use. Caution with infant renal impairment or premature infants due to potential accumulation. Use only if necessary.

Pregnancy Dosing
INJECTAPAP

No dose adjustment required for standard therapeutic use. Increased clearance in pregnancy may require shorter dosing intervals for pain control; consider maximum daily dose of 3 g/day instead of 4 g/day. Avoid prolonged use >48 hours without medical supervision.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Increased renal clearance in pregnancy may lower serum levels; consider higher doses based on therapeutic drug monitoring. Adjust for renal impairment if present. Standard initial dosing: 15 mg/kg/day IV/IM divided q8-12h, with level-guided adjustments.

Clinical Pearls
INJECTAPAP

Acetaminophen injection is indicated for treatment of acute pain and fever. Use with caution in hepatic impairment. Avoid in patients with severe active liver disease. Monitor liver function tests with prolonged use. Do not exceed maximum daily dose (4 g/day in adults). Use the smallest effective dose for the shortest duration.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Amikacin is an aminoglycoside antibiotic with concentration-dependent bactericidal activity. Monitor peak (20-30 mcg/m L) and trough (<10 mcg/m L) serum levels to optimize efficacy and minimize toxicity. Adjust dose based on renal function (Cr Cl). Ototoxicity (vestibular and cochlear) and nephrotoxicity are dose-limiting; audiometry and renal function tests are mandatory. Extended-interval dosing (15-20 mg/kg once daily) is preferred for most indications. Avoid concurrent use with other nephrotoxic drugs (e.g., vancomycin, loop diuretics).

Patient Counseling
INJECTAPAP

Do not take more than the recommended dose. Overdose can cause severe liver damage.,Inform your healthcare provider if you have liver disease or drink alcohol regularly.,Check other medications for acetaminophen to avoid double dosing.,Seek immediate medical attention if you experience signs of liver injury (e.g., yellowing skin/eyes, dark urine, upper stomach pain).,This medication is administered by intravenous infusion; do not attempt self-administration.

AMIKIN IN SODIUM CHLORIDE 0.9% IN PLASTIC CONTAINER

Take exactly as prescribed; do not skip doses or stop early.,Drink plenty of fluids to stay hydrated.,Report hearing changes (ringing in ears, dizziness) immediately.,Report decreased urine output or swelling in legs.,Avoid taking other medications without consulting your doctor, especially pain relievers like ibuprofen.,This medication is given intravenously; you may feel warmth or tingling during infusion.
