Mechanism of Action
ISMOTIC

Isosmotic solution of mannitol; increases plasma osmolality, drawing water from tissues into the vasculature and reducing intracranial/intraocular pressure via osmotic diuresis.

ACETAMINOPHEN AND CODEINE PHOSPHATE

Acetaminophen: centrally acting analgesic and antipyretic, possibly via inhibition of cyclooxygenase (COX) and modulation of cannabinoid receptors. Codeine: prodrug converted to morphine; mu-opioid receptor agonist.

Avoid high-tyramine foods (aged cheeses, cured meats, soy products) as hydralazine may increase tyramine sensitivity? No significant specific food interactions for isosorbide dinitrate/hydralazine. Teratogenic Risk
ISMOTIC

In animal studies, administration of isosorbide dinitrate (active ingredient of Ismotic) during organogenesis produced fetal toxicity at doses 35 times the maximum human dose. First trimester: unknown risk, avoid unless clearly needed. Second and third trimesters: risk of maternal hypotension and reduced placental perfusion; use only if potential benefit justifies risk. Should be used with caution near term due to risk of neonatal hypotension.

ACETAMINOPHEN AND CODEINE PHOSPHATE

Acetaminophen is considered low risk in all trimesters at therapeutic doses; chronic high doses may be associated with adverse outcomes. Codeine is associated with risk of respiratory depression and neonatal withdrawal if used near term; may cause neural tube defects and other malformations with first-trimester exposure, but data are conflicting. Use lowest effective dose for shortest duration.

Lactation Summary
ISMOTIC

Isosorbide dinitrate is excreted in human breast milk; clinical significance unknown. M/P ratio not reported. Caution is advised; consider temporary discontinuation of breastfeeding during therapy.

ACETAMINOPHEN AND CODEINE PHOSPHATE

Acetaminophen is excreted into breast milk in low amounts (M/P ratio ~0.91-1.42) and is considered compatible with breastfeeding. Codeine is also excreted in breast milk; risk of infant opioid toxicity depends on maternal CYP2D6 phenotype. Ultra-rapid metabolizers may produce higher morphine levels. Use with caution, avoid in known CYP2D6 ultra-rapid metabolizers, and monitor infant for sedation and respiratory depression.

Avoid use with PDE-5 inhibitors (e.g., sildenafil) due to risk of severe hypotension. Titrate gradually to target dose to minimize adverse effects. May cause drug-induced lupus-like syndrome or peripheral neuropathy with hydralazine; consider slow acetylator phenotype risk.
