Mechanism of Action
ISOLYTE E IN PLASTIC CONTAINER

ISOLYTE E is an intravenous electrolyte replacement solution that provides water, electrolytes (sodium, potassium, magnesium, calcium, chloride, acetate, and gluconate), and bicarbonate precursors to correct fluid and electrolyte imbalances. The acetate and gluconate ions are metabolized to bicarbonate in the liver, providing an alkaline buffer.

METHERGINE

Methylergonovine is an ergot alkaloid that acts as a partial agonist at α-adrenergic receptors in the uterine smooth muscle, causing sustained contractions. It also exhibits serotonergic (5-HT2) and dopaminergic activity.

Standard Dosing

Intravenous infusion; rate and volume determined by individual patient requirements for fluid and electrolyte replacement. Typical adult dose: 500-1000 m L as a single infusion, administered at a rate of 5-10 m L/min.

First trimester: potential teratogenic effects not well studied; avoid use. Second and third trimesters: can cause abruptio placentae, premature labor, and fetal anoxia. It is FDA Pregnancy Category X.

Caution in patients with hyperkalemia, renal failure, or metabolic alkalosis. Do not administer simultaneously with blood products due to risk of hemolysis. Observe for signs of fluid overload in patients with heart failure.

METHERGINE

METHERGINE (methylergonovine) is an ergot alkaloid used primarily for postpartum hemorrhage due to uterine atony. Do not use for routine induction of labor or for threatened abortion. Avoid in patients with hypertension, preeclampsia, coronary artery disease, or severe hepatic/renal disease. Monitor blood pressure closely during administration. Administer intramuscularly for rapid effect; onset is 2-5 minutes. Intravenous administration should be reserved for emergencies due to risk of hypertensive crisis.
