ISOSULFAN BLUE vs LYMPHAZURIN

Isosulfan blue is a dye that binds to serum proteins after intradermal injection, allowing visualization of lymphatic channels and lymph nodes during sentinel lymph node biopsy. It has no known pharmacological activity beyond its use as a visual contrast agent.
Lymphazurin is a diagnostic dye composed of isosulfan blue. It is a hydrophilic molecule that selectively binds to serum albumin and is preferentially taken up by lymphatic vessels. When injected subcutaneously or intradermally, it enters the lymphatic system and stains the lymphatic vessels and nodes, allowing for visual identification during lymph node mapping and sentinel lymph node biopsy procedures.
1-2 mL (10-20 mg) of a 1% solution injected subcutaneously or intradermally around the tumor or into the tumor, typically 3-5 minutes prior to sentinel lymph node biopsy.
0.1 mL (100 mcg) administered via submucosal injection in 3 divided doses into the distal submucosa surrounding the cervical os (12, 4, and 8 o'clock positions) at a depth of 3 mm, 10 minutes prior to the procedure.

Terminal elimination half-life is approximately 20-30 minutes. Clinically, the dye is rapidly cleared from the plasma, which correlates with its short duration of action as a lymphographic agent.
Terminal elimination half-life is 18–24 hours; prolonged in renal impairment (up to 40 hours in severe impairment), requiring dose adjustment.
Primarily excreted unchanged in bile via the hepatobiliary system (approximately 90%). Renal excretion accounts for less than 10% of the administered dose.
