Mechanism of Action
MAGNESIUM HYDROXIDE AND OMEPRAZOLE AND SODIUM BICARBONATE

Magnesium hydroxide is an antacid that neutralizes gastric acid, increasing gastric p H. Omeprazole is a proton pump inhibitor (PPI) that irreversibly inhibits the H+/K+ ATPase enzyme system at the secretory surface of gastric parietal cells, blocking the final step of acid secretion. Sodium bicarbonate is a systemic antacid that neutralizes gastric acid and also provides alkalinization of urine.

OMEPRAZOLE AND SODIUM BICARBONATE

Omeprazole is a proton pump inhibitor that suppresses gastric acid secretion by inhibiting the H+/K+ ATPase enzyme system at the secretory surface of gastric parietal cells. Sodium bicarbonate is an antacid that neutralizes gastric acid.

Standard Dosing
MAGNESIUM HYDROXIDE AND OMEPRAZOLE AND SODIUM BICARBONATE

One tablet (containing 400 mg magnesium hydroxide, 20 mg omeprazole, 1000 mg sodium bicarbonate) orally once daily, taken at least 1 hour before a meal.

OMEPRAZOLE AND SODIUM BICARBONATE

Omeprazole 20 mg plus sodium bicarbonate 1100 mg orally once daily before a meal; for gastroesophageal reflux disease, dose may be increased to 40 mg orally once daily for 4-8 weeks.

Metabolism

Omeprazole is extensively metabolized in the liver by cytochrome P450 (CYP) enzymes, primarily CYP2C19 and CYP3A4, to inactive metabolites. Sodium bicarbonate is not metabolized; it dissociates into sodium and bicarbonate ions.

Omeprazole is primarily metabolized by CYP2C19 and CYP3A4; metabolites are excreted renally (~77% as metabolites) and fecally (~20% as metabolites). Urinary excretion of unchanged omeprazole is negligible (<1%). Sodium bicarbonate is excreted renally as bicarbonate and carbon dioxide.

OMEPRAZOLE AND SODIUM BICARBONATE

Oral bioavailability is approximately 30-40% after a single dose, increasing to 60-70% with repeated administration due to decreased first-pass metabolism. Bioavailability is not affected by food but is enhanced by the sodium bicarbonate component, which protects omeprazole from acid degradation.

Renal Adjustments
MAGNESIUM HYDROXIDE AND OMEPRAZOLE AND SODIUM BICARBONATE

Contraindicated in severe renal impairment (e GFR <30 m L/min/1.73m²) due to risk of magnesium accumulation and sodium overload. For e GFR 30-59 m L/min/1.73m², reduce dose to one tablet every other day and monitor serum magnesium and sodium.

OMEPRAZOLE AND SODIUM BICARBONATE

No dosage adjustment required for mild to moderate renal impairment; for severe renal impairment (GFR <30 m L/min), use with caution and monitor for sodium overload.

Hepatic Adjustments
MAGNESIUM HYDROXIDE AND OMEPRAZOLE AND SODIUM BICARBONATE

Child-Pugh A: No adjustment. Child-Pugh B: Reduce omeprazole dose to 10 mg (not available in this combination) or consider alternative; use with caution. Child-Pugh C: Contraindicated due to omeprazole accumulation.

Long-term use (≥1 year) may increase risk of osteoporosis-related fractures; hypomagnesemia with prolonged PPI use; cyanocobalamin (vitamin B12) deficiency with long-term acid suppression; magnesium hydroxide may cause diarrhea; sodium bicarbonate may cause metabolic alkalosis, fluid retention, and worsen hypertension or heart failure; acute interstitial nephritis reported with PPIs; monitor renal function; interaction with clopidogrel (omeprazole reduces clopidogrel's active metabolite); increased risk of Clostridium difficile infection; avoid concurrent use of atazanavir or nelfinavir.

Second and third trimesters: Omeprazole is considered low risk; magnesium hydroxide can cause hypotonia and respiratory depression in neonates with prolonged use; sodium bicarbonate may lead to fluid overload or alkalosis.

OMEPRAZOLE AND SODIUM BICARBONATE

First trimester: No increased risk of major congenital malformations based on large cohort studies. Second and third trimesters: Limited data, but no evidence of fetal harm. Omeprazole is FDA Pregnancy Category C; sodium bicarbonate is not associated with teratogenicity.

Lactation Summary

Omeprazole is excreted in breast milk in low amounts (M/P ratio ~0.5); magnesium hydroxide and sodium bicarbonate are poorly absorbed; considered compatible with breastfeeding, but monitor infant for diarrhea or electrolyte imbalance.

OMEPRAZOLE AND SODIUM BICARBONATE

Omeprazole is excreted into breast milk with an M/P ratio of approximately 0.1-0.2. Sodium bicarbonate is also excreted. At therapeutic doses, amounts are unlikely to affect the infant. Manufacturer advises caution, but use is generally considered compatible with breastfeeding.

MAGNESIUM HYDROXIDE AND OMEPRAZOLE AND SODIUM BICARBONATE

This combination uses sodium bicarbonate to rapidly raise gastric p H, enabling omeprazole absorption (enteric-coated omeprazole may be prematurely released; use non-enteric formulations). Magnesium hydroxide provides additional acid neutralization and a laxative effect. Avoid in patients with renal impairment (risk of magnesium toxicity, sodium overload). Administer on an empty stomach at least 1 hour before meals. Do not split or crush tablets.

OMEPRAZOLE AND SODIUM BICARBONATE

Administer on an empty stomach 1 hour before a meal for maximal acid suppression. The sodium bicarbonate component provides rapid antacid effect and may cause belching or gastric distension. Avoid in patients with Bartter's syndrome, hypokalemia, or metabolic alkalosis. Monitor magnesium levels with prolonged use; hypomagnesemia can occur with PPIs. For patients unable to swallow capsules, the contents can be mixed with applesauce.
